Undergraduate Forms

The forms listed below are submitted by students as a means of appealing SCI policies, adjusting enrollment, and requesting changes to their student record.  Web forms, including DocuSign Power Forms, will be automatically directed to the appropriate recipient(s) and signer(s).

I need to...

Once the Add/Drop period has ended, you have until the grade option change deadline to either change your grade option or audit the course.  You would submit the Grade Option/Audit Request form to the school offering the course.

If you are auditing the course, please be advised that you must be registered for the course you are auditing, and tuition is charged as normal for audited courses.  For more information about payments, please reach out to the Student Payment Center 

If you are student in the School of Computing and Information (SCI) and would like to add or change your major in SCI, please complete the SCI Undergraduate Major Declaration Form. Any declaration forms received before or during the Add/Drop period of term will be processed effective for that term.  Please note, due to the number of forms received and processing times, you may not see the change reflected on your record for a couple of weeks after the Add/Drop period has passed.  Declarations received after Add/Drop will be processed for the following semester.

If you are not currently a student in SCI and want to declare an SCI major, you must first apply and be accepted as an SCI internal transfer or double degree student.  Please reach out to your home school for the correct form to start this process.  You can find more information on admission requirements on the SCI Undergraduate Admissions page 

To emphasize a multidisciplinary approach, SCI undergraduate students are required to have a minimum of two areas of focus.  This requirement can be satisfied by:

  • Completion of a jointly offered major with another school on campus
  • Completion of a minor or second major
  • an approved certificate program

If none of the options fit your situation, you will need to work with your advisor to build an area of focus and submit this Secondary Field of Study (SFoS) Declaration. If you are unsure if this requirement is fulfilled, reach out to your advisor.

To declare or change a Minor within SCI, fill out the Undergraduate Minor Declaration/Change Form.  Any addition or change to a Minor will be active within the term.  Please be advised that Minors should be declared before a student applies for Graduation.

If a class is approved to fulfill two General Education Requirements and you want to change the requirement that the class fulfills, complete a Best Fit Form.  Be advised that if the course you submit for a best fit is currently in progress, it will not show up in your student record until a grade has been posted.  This form will not allow a course to count towards a general education requirement without formal approval. 

If you would like to appeal for a course already in your record to fulfill a GER with a course that is not currently approved, please review the General Education Requirement appeal information found on the Student Appeals page and reach out to your Advisor to initiate the process.

An Enrollment Worksheet is required supplemental material when a student is submitting a Late Enrollment petition.  This worksheet is used to document a proposed enrollment plan.  Please see the Late Enrollment section of the Student Appeals page for more information.

If you have credits you have transferred into the University or have taken a course taken at Pitt that could fit the spirit of a General Education requirement, you should work with your Advisor to put together a petition for review. 

After the add/drop period has ended, you may withdraw from a course by completing a Monitored Withdrawal Request form.  Submit this form for all SCI courses no later than the monitored withdrawal deadline published in the University’s Academic Calendar for the term. All forms must be received by 5:00 PM Eastern Time. 

 NOTICE: Starting in the Fall 2025 term, withdrawals from a course now count as an official attempt. This means that they count towards the number of repeats you have when taking a course.  Students can only repeat a course twice, for a total of three attempts.  You can see the policy update here: https://www.provost.pitt.edu/course-repeat.  For questions about the impact of a Withdrawal on your record, please reach out to your advisor.

To withdraw from SCI courses (courses with CMPINF, CS, INFSCI subject codes), fill out this form.

For All Other Pitt courses, fill out this form.

Undergraduate students can cross-register for courses through the Pittsburgh Council on Higher Education (PCHE) program.  Students can be cross registered between their home school and another participating school to take courses not offered at their home institution.  Students who are interested in enrolling in a course at a PCHE institution must submit the advisor signed PCHE Cross Registration Form to SCI Academic Records by the deadline determined by the University of Pittsburgh Registrar's office.  Once the form is submitted, Academic Records will obtain the required Dean’s signature and route to the Registrar’s Office for final processing.

A Permission Number is a code used to override an enrollment restriction on a course. This code is issued by the school or department offering the course. More information on submitting a Permission Number Request can be found in the SCI Permission Numbers Instructions. Please read and follow the instructions on the Permission Number Application carefully. Approval for requests can only be granted approval if all requirements are met.

Academic Records automatically submits the Registrar’s Course Repeat Form for students who have repeated the exact same course (i.e., repeating INFSCI 0017). However, any student who is replacing a course with an established comparable one (i.e., replacing INFSCI 0017 with CMPINF 0401), must submit a Course Repeat Form to their Advisor PRIOR to enrollment. The advisor will review course repeat policies with the student and submit the form to Records on behalf of the student at the end of the term. 

A student can request a change to their Requirement Term once during their Academic career.  The requirement term must apply to General Education and Major requirements.  Reach out to your Advisor to discuss the process.

The SCI Undergraduate Transfer Credit Request Form is for current SCI students looking to take courses at another institution and transfer the credit back to their University of Pittsburgh record.

Students are eligible to bring in transfer credit during their time at Pitt if they meet the following requirements:

  • Student is in good academic standing
  • Student has not completed 90 credits of course work prior to the transfer of the away course
  • Student has not previously completed proposed course work at the University of Pittsburgh

Questions about eligibility should be discussed with your advisor.

Forms will be accepted beginning after add/drop in the term before you are taking the course. i.e., if you plan to take a course at another school in Summer 2026, the earliest you can submit the form is after add/drop in Spring 2026. 

After a transfer course has been evaluated, a student can request that the course be re-evaluated using the SCI Undergraduate Transfer Credit Reevaluation Form.  Please make sure to attach any supporting documentation such as the class syllabus and/or course work completed within the class.

The Transfer Out/Dual Degree form is a declaration of either 1) Transferring out of SCI or 2) Adding a Dual Degree to your student record. Admission into another School at the University of Pittsburgh is at the discretion of the receiving School.
